Sonoma County Early Relational Health Consortium

Together, we are working to build bridges across sectors and services to create a continuum of care that is responsive, equitable, seamless, and effectively meets the dyadic needs of the most marginalized birthing people, children 0-3 and their families.

The Consortium defines early relational health (or “ERH”) as the everyday interactions between a child and caregiver, which are shaped by the environment, that help the child and parent thrive together.

First 5 Sonoma County’s Role

Through Measure I, First 5 Sonoma County has the ability to serve as a backbone and convene this cross-sector consortium.

Our Collaborative Work

Since November 2025, the Consortium has met five times to:
  • Identify priority areas
  • Brainstorm and prioritize strategies that build upon the great network of resources that already exist
  • Establish measures of success
  • Set community practices and explore ways to advance the work together
All of these discussions have informed a Working Roadmap that will help guide our collective efforts.

How To Get Involved

  • Interested organizations are invited to join the Consortium and attend our quarterly meetings to help build bridges between the work your agency is implementing with other local partners and share your perspective and ideas.
